Support Coordination

NDIS Support Coordination across Greater Sydney

A support coordinator helps you understand your NDIS plan, find the right providers and make your funding work for your goals. At Capable Life, we also help you build the skills and confidence to manage more of your plan yourself over time.

We are a small, founder-led team. You work with the same coordinator from start to finish.

What we do

  • Explain your plan in plain language: what each budget is for and how much you have.
  • Find and connect providers that suit your goals, needs and preferences, and give you real choice.
  • Set up service agreements and bookings, and check your supports are working.
  • Link you with community and mainstream services, such as health, education, housing and local groups.
  • Help when things change or go wrong, such as a provider leaving, a change in health or a crisis.
  • Prepare for your plan reassessment with progress reports and evidence of what you need.
  • Build your own skills to understand your plan, contact providers and make decisions, using clear, step-by-step goals.

Who we work with

  • Children, young people and adults with disability, including neurodivergent and autistic people and people with intellectual disability
  • People with psychosocial disability
  • School leavers and young adults moving to more independence
  • Families and carers who want one clear point of contact

How it works

  1. Referral: you, your family, your planner or LAC sends a referral or calls us.
  2. First conversation: we talk about your goals and check we are the right fit.
  3. Service agreement: we explain our service and costs clearly before anything starts.
  4. Getting started: we meet you at home or somewhere you feel comfortable, review your plan, and make a step-by-step action plan together.
  5. Ongoing support: regular check-ins, help with any problems, and a progress report before your plan review.

Areas we cover

We support participants across Greater Sydney. Not sure if we cover your area? Just ask.

Funding

Support coordination is paid from the Support Coordination budget in the Capacity Building section of your NDIS plan. We charge within the NDIS Pricing Arrangements and Price Limits, so there is no out-of-pocket cost to you. We work with agency-managed, plan-managed and self-managed participants.

Frequently asked questions

How do I know if support coordination is in my plan?

Look in the Capacity Building section of your plan for “Support Coordination”. If you’re not sure, send us your plan details through the referral form or call us and we’ll check with you.

Can I change support coordinators?

Yes. You can change at any time by giving notice to your current provider, as set out in your service agreement. We can help make the handover smooth.

Do you provide other services?

Yes. We also provide capacity building (life skills coaching). You never have to use our other services to use our support coordination. Read our Conflict of Interest statement to see how we protect your choice.

What is happening to support coordination in 2028?

The Australian Government has announced that from 1 July 2028, support coordination will be delivered through a new service with providers appointed by the NDIA, instead of being funded in individual plans. Until then, nothing changes, and we will keep you informed and help you through any transition.

What if I’m unhappy with the service?

Tell us. See our Feedback and Complaints page for how we handle concerns and who else you can contact.
